== Career == Chilkoti joined Duke University in 1996 as an assistant professor of biomedical engineering. He was promoted to associate professor in 2002 and professor in 2006. He was the Theo Pilkington Chair Professor of Biomedical Engineering from 2008 to 2013, and has been the Alan L. Kaganov Distinguished Professor of Biomedical Engineering at Duke University since 2013. From 2002 till 2007, Chilkoti served as associate director of the Center for Biologically Inspired Materials and Material Systems at Duke University and was the director of the center from 2007 to 2011. In 2014, he became Chair of the Department of Biomedical Engineering at Duke University and served as chair until 2022. As Chair of the Duke BME department, he launched an incubator—BRiDGE—for Biomedical Engineering faculty and student startups, and he created the Duke Engineering Entrepreneurship program (DEEP)—a post-doctoral fellowship program for recent PhD graduates of the department interested in entrepreneurship. Since 2023, he has been serving as the senior associate dean of Pratt School of Engineering at Duke University. Chilkoti founded two Gordon Research Conferences (GRCs)—one on Biointerface Science in 2006 and a second on Bioinspired Materials in 2012. Furthermore, he is the founder of five start-ups.

=== Acetals and hemiacetals === Hemiacetals and acetals are essentially tetrahedral intermediates. They form when nucleophiles add to a carbonyl group, but unlike tetrahedral intermediates they can be very stable and used as protective groups in synthetic chemistry. A very well known reaction occurs when acetaldehyde is dissolved in methanol, producing a hemiacetal. Most hemiacetals are unstable with respect to their parent aldehydes and alcohols. For example, the equilibrium constant for reaction of acetaldehyde with simple alcohols is about 0.5, where the equilibrium constant is defined as K = [hemiacetal]/[aldehyde][alcohol]. Hemiacetals of ketones (sometimes called hemiketals) are even less stable than those of aldehydes. However, cyclic hemiacetals and hemiacetals bearing electron withdrawing groups are stable. Electron-withdrawing groups attached to the carbonyl atom shift the equilibrium constant toward the hemiacetal. They increase the polarization of the carbonyl group, which already has a positively polarized carbonyl carbon, and make it even more prone to attack by a nucleophile. The chart below shows the extent of hydration of some carbonyl compounds. Hexafluoroacetone is probably the most hydrated carbonyl compound possible. Formaldehyde reacts with water so readily because its substituents are very small- a purely steric effect.

== Prelude == In November 2024, a coalition of rebel groups spearheaded by HTS attacked Aleppo, quickly overrunning the demotivated and disorganized defenders. In response, Assad travelled to Russia to ask for military aid on 28 November; his request was denied. Upon returning home, however, he lied to his senior commanders and advisors, claiming that Russia would soon send aid. His wife Asma had already been in Moscow for cancer treatment for months, accompanied by their children. A few days later, the rebels captured Aleppo. Afterward, reports emerged of a coup led by Syrian State Security Director General Hossam Louka against the Assad government. The Syrian Army General Command as well as the Iranian ambassador to Syria, Hossein Akbari, both denied these reports. On 2 December, Assad met with Iranian Foreign Minister Abbas Araghchi to discuss the situation. He confided that the Syrian Armed Forces were too weak to offer effective resistance to the continuing rebel advance. Regardless, he did not request increased Iranian aid, probably due to concerns about an Israeli intervention. At this point, the Syrian president had possibly concluded that the war was already lost. Despite this, he continued to tell his inner circle that Russian support would be coming, imploring them to continue their work. After the fall of Hama on 5 December, the government began redeploying its troops from Eastern Ghouta, including Douma and Harasta, to the entrances and exits of Damascus.

== History == The first 15 years of the EIP focused on establishing vaccination schedules and ensuring that poorer countries had a supply of vaccines. In 1974, only 5% of children in low-income countries and 21% of all children received basic vaccines. DTP3 coverage increased from 20% in 1982 to 76% in 1990. (DTP3 refers to the third shot of the DTP vaccine. DTP3 completion rates in one-year-olds is used as a proxy for infant vaccination completion rates.) The growth was fueled by Child Survival and Development Revolution initiated by Jim Grant in UNICEF. After the sharp increase in vaccination rates, the growth has slowed to a halt because the most of the remaining infants live in places where the cold chain for vaccine storage is unreliable. In response the EPI broadened its scope and introduced more vaccines (HepB, Hib, PCV) into schedules. This expands the spectrum of protection for kids with access to vaccines. The GAVI was founded in 2000, providing funding for expanding vaccine access in poorer countries. The DTP3 completion rate was 83% by 2010. Technical innovations such as the solar-powered refrigerator and the vaccine vial monitor played a large role in gaining the 7 percentage points. In the one-and-a-half decades after 2010, the EPI saw major achievements including the appearance and endorsement of rotavirus, HPV, and malaria vaccines. The rotavirus vaccine greatly reduced women and child mortality due to diarrhea; the HPV vaccine had already made a dent in the number of cervical cancer cases; and the malaria vaccine too has saved lives.

== Clinical significance == Due to the essential role of NADPH in lipid and DNA biosynthesis and the hyperproliferative nature of most cancers, NADK is an attractive target for cancer therapy. Furthermore, NADPH is required for the antioxidant activities of thioredoxin reductase and glutaredoxin. Thionicotinamide and other nicotinamide analogs are potential inhibitors of NADK, and studies show that treatment of colon cancer cells with thionicotinamide suppresses the cytosolic NADPH pool to increase oxidative stress and synergizes with chemotherapy. While the role of NADK in increasing the NADPH pool appears to offer protection against apoptosis, there are also cases where NADK activity appears to potentiate cell death. Genetic studies done in human haploid cell lines indicate that knocking out NADK may protect from certain non-apoptotic stimuli.
